Jack E. Chezem, age 66, of Melrose died Monday, February 28, 2005 at the St. Cloud Hospital.
Memorial services will be held at 11 a.m. Thursday, March 3 at the Patton-Schad Funeral Home in Melrose with Deacon Ernie Kociemba officiating. Inurnment will follow at St. Mary's Cemetery in Melrose with military honors by the Melrose Veterans Honor Guard.
Friends may call one hour prior to the service at the funeral home in Melrose.
Jack E. Chezem was born February 18, 1939 in Houston, Texas to Merle and Mary (Cooper) Chezem. He was a veteran serving in the U.S. Navy from 1960 to 1962. Jack was employed for 17 years retiring as a truck driver for DuPont. They then moved to Melrose. He enjoyed NASCAR and was a big Jeff Gordon Fan. Jack liked boating, fishing, and listening to Cher's music.
He is survived by his children, Michael Chezem of Melrose, Mark Chezem of LaPorte, Texas, and Sheryl Smallwood of Kerrville, Texas; five Grandchildren, Mike, Carli and Chelsi Chezem and Nichole and Rachel Gagne; and sister, Beth Ann Baker (Billy) of South Houston, Texas.
Jack was preceded in death by his wife, Madonna M. Chezem on December 7, 2000; and sisters, Alma Dean Barlow and Frankie Vickery.
Urn bearer will be his son, Michael Chezem.
